Are you getting the most out of your Home Care Package?

CENTACARECQ has been a Home Care Package Provider in Central Queensland for more than 20 years.

The Home Care Package program helps residents to live an independent life in their own home for as long as they can.

The program provides a subsidy towards a package of care, services and case management to meet individual needs.

It is about control and choice in the way each service is delivered.

In 2017, the Department of Health introduced Increasing Choices Reforms in regards to the way Home Care was being delivered.

These changes have ensured that:

• Funding for a Home Care Package will now follow the client, not the organisation.

• Funding for a Home Care Package will now follow the client, not the organisation.

• Clients will be able to change their provider if they wish. This also includes if the client moves to another area to live.

• There will be a consistent national approach to prioritising access to home care through the My Aged Care gateway.

• There is a fairer and more flexible ways of distributing packages to clients based on individual needs and circumstances, regardless of where they live.

Home Care Package Levels

The Home Care Packages Program provides four levels of support:

• Home Care Level 1- basic care needs

• Home Care Level 2 – low level care needs

• Home Care Level 3 – intermediate care needs

• Home Care Level 4- high care needs

Home Care Package clients are not limited to a basic list of services.

Clients can use their Home Care Package funds to purchase a wide range of services including:

• Clinical care, such as nursing, allied health and physiotherapy for mobility and strength;

• Clinical care, such as nursing, allied health and physiotherapy for mobility and strength;

• Support services, such as help around the home, visiting the doctor and attending social activities;

• Personal care, such as help with showering, dressing and moving around the home; and

• Care coordination and Case Management.

Subcontracting services

SHOULD you not be able to access a service that is provided by CentacareCQ we can subcontract out the service you require to ensure your needs are met.

• CentacareCQ is dedicated to work in partnership with you to tailor care services to best support your needs and goals.

• The care and services in which we provide to you are based on your assessed care needs. This is determined by the Aged Care Assessment Team.

• Home Care Package funds cannot be used as a general source of income for items such as day-to day bills, care registration,mortgage payments or rent as an example.

Our friendly experienced team can assist clients in navigating their way around the My Aged Care gateway which in turn help you receive a relevant assessment by a professional assessor who will determine the right level of service for you.
